A RESOLUTION approving a group dental plan and rates as a County-provided benefit at an increased rate of three percent (3%) for a Dental Health Maintenance Organization (DHMO) plan and seventeen percent (17 %) for two indemnity plans provided by Assurance Employee Benefits (formerly known as Fortis Benefits).

Body
WHEREAS, Fortis Benefits was awarded the bid for group dental insurance for County employees for the 2003 plan year and the award was extended for the 2004 plan year with no rate increase by Resolution 14746, dated October 6, 2003; and,
WHEREAS, Assurant Employee Benefits (formerly Fortis Benefits) has provided group dental insurance at a low cost to employees and the County for many years; and,
WHEREAS, Assurant Employee Benefits has proposed an extension for the 2005 plan year with a three percent (3%) rate increase in the cost for the DHMO plan and a seventeen percent (17%) increase for the indemnity plans; and,
WHEREAS, the Administration and the County's dental broker, Bill Egelhoff, recommend the extension for the 2005 plan year with the following group dental plans and rates:

BE IT RESOLVED by the County Legislature of Jackson County, Missouri, that the County Executive is hereby authorized to execute an agreement with Assurance Employee Benefits (formerly known as Fortis Benefits) for employee group dental insurance coverage with the following group dental plans and rates:

and to execute any and all documents necessary to effect this extension and renewal in a form to be approved by the County Counselor.

Effective Date: This Resolution shall be effective immediately upon its passage by a majority of the Legislature.
