Body
WHEREAS, the Purchasing Director has solicited formal written proposals on Request for Proposals No. 6-04 for the furnishing of urinalysis testing for use by the Prosecuting Attorney's Drug Court Deferred Prosecution Program to provide for departmental needs for the upcoming two-year period; and,

WHEREAS, he has received proposals from the following:
VENDOR

Kroll Laboratory Specialist, Inc.
Gretna, LA

Health Productivity
Kansas City (Jackson County), MO

Norchem Drug Testing
Flagstaff, AZ

and,

WHEREAS, the Health and Justice Committee heard testimony about the proposed contracts on November 8, 2004, and is satisfied that Health Productivity has submitted the lowest and best bid; and,

WHEREAS, the Prosecuting Attorney recommends that the award be made to Health Productivity of Kansas City, MO, for the reason that it has submitted the lowest and best bid; therefore,

BE IT RESOLVED by the County Legislature of Jackson County, Missouri, that award be made to Health Productivity of Kansas City, MO, in the estimated annual amount not to exceed $103,350.00 as recommended by the Prosecuting Attorney, and that the Purchasing Director be, and is hereby, authorized to execute for the County any documents necessary to the accomplishment of the award; and,

BE IT FURTHER RESOLVED that the Manager of the Division of Finance is authorized to ...
