Body
WHEREAS, by Resolution 10762, dated April 3, 1995, a term-and-supply contract with Health Productivity Systems of Kansas City, MO, for the furnishing of urinalysis testing for use by the Prosecuting Attorney's Deferred Prosecution and Drug Court programs was approved; and,

WHEREAS, by Resolution 11363, dated July 22, 1996, Resolution 11762, dated May 19, 1997, and Resolution 12006, dated January 12, 1998, increases to this contract were made in the respective amounts of $100,000.00, $100,000.00, and $54,000.00; and,

WHEREAS, it is necessary to further increase this contract in the amount of $45,400.00 in order to allow Health Productivity Systems to continue providing urinalysis services to participants of the Deferred Prosecution and Drug Court programs through July 31, 1998; and,


WHEREAS, the Purchasing Director has recommended the increase to this contract upon the terms and conditions of the Fourth Addendum attached hereto; now therefore,

BE IT RESOLVED by the County Legislature of Jackson County, Missouri, that the increase to the term-and-supply contract be made as recommended by the Purchasing Director, and that the Purchasing Director be, and is hereby, authorized to execute for the County any documents necessary to the accomplishment of the increase; and,

BE IT FURTHER RESOLVED that the Director of the Department of Finance is authorized to make all payments, including final payment on the contract, to the extent sufficient appropriations to the using spending agency have heretofore ...
