Body
WHEREAS, by Resolution 12950, dated November 22, 1999, the Legislature authorized a contract with Health Productivity Systems of Kansas City, MO, for the furnishing of urinalysis testing for use by the Prosecuting Attorney's Deferred Prosecution and Drug Court programs; and,

WHEREAS, by a First Addendum to Agreement dated September 5, 2000, the terms of said contract were extended; and,

WHEREAS, the Legislature did, by Resolution 13657, dated April 9, 2001, authorize a Second Addendum to the Agreement, increasing the contract in the amount of $160,000.00 and extending the terms of the Agreement for an additional twelve months; and,

WHEREAS, by Resolution 14260, dated July 15, 2002, the Legislature did authorize a Third Addendum to the Agreement, increasing the contract in the amount of $14,500.00 and extending the terms of the Agreement; and,

WHEREAS, by Resolution 14320, dated September 16, 2002, the Legislature did authorize a Fourth Addendum to the Agreement, increasing the contract in the amount of $30,000.00 and extending the terms of the Agreement; and,

WHEREAS, the Prosecuting Attorney has recommended an increase to this contract in the amount of $24,573.00, in order to allow Health Productivity Systems to continue providing urinalysis services to participants of the Deferred Prosecution and Drug Court programs; and,

WHEREAS, the attached Fifth Addendum to Agreement provides a suitable mechanism by which to implement the Prosecuting Attorney's recommendation in this r...
