Body
WHEREAS, CBIZ Benefits of Kansas City, MO has solicited new written proposals for a health coaching and lifestyle management program for County employees; and,
WHEREAS, after evaluation of the proposals received, CBIZ has recommended the award of employee health coaching and lifestyle management insurance to Tria Health of Overland Park; KS; and,
WHEREAS, Tria Health has previously supplied individual plans through their company and now upon the recommendation of CBIZ, Tria Health is offering group plans to Jackson County associates; and,
WHEREAS, after evaluation of the proposals received, CBIZ Benefits has recommended the award of employee health coaching and lifestyle management, focused solely on weight loss to Tria Health of Overland Park, KS; and,
WHEREAS, pursuant to section 1054.6 of the Jackson County Code, 1984 the Interim Director of Human Resources recommends the award of a twelve-month term and supply contract for the furnishing of an employee health coaching and lifestyle management program focused solely on weight loss to the providers recommended by CBIZ; and,
WHEREAS, the total monthly premium costs of the recommended plans for 2027 are determined by the number of associates that elect the plan, currently associates that are signed up for the plan will incur a cost of a $150 co-pay for their participation; and,
WHEREAS, the total monthly premium costs of the recommended group health coaching and lifestyle management insurance will be based upon age and coverage amount per participant; and,
